Terms & Conditions for Monthly Massage Subscription

The Initial Subscription period shall be for a period of 12 months (The Initial Period). Subscription fee will be charged to the designated credit card once per month for 12 months. The subscriber agrees to pay the total cost of the 12 sessions in monthly installments. The subscriber may cancel the subscription at any time by giving a 30-day written notice. Upon early cancellation, the subscriber’s credit card will be charged a $25 cancellation fee plus the amount that was discounted for each session previously billed within the canceled Initial Period. Any unused services will expire 90 days after you elect to cancel your subscription.

After the Initial Period, the subscription may be renewed by purchasing a new subscription. Any unused services will expire 90 days after the Initial Period unless a new subscription has been purchased.

There may be times when extenuating circumstances (such as extended illness, military service, or temporary relocation) cause a member to need to suspend their subscription. A subscriber may suspend their subscription for up to a total of 3 months during the Initial Period by providing written notice 30 days prior to the beginning of the date the suspension is to begin. This will extend the Initial Period by the number of months the subscription is suspended. (i.e. from 12 months to a maximum of 15 months if suspended for 3 months). During a period of subscription suspension, your account will not be charged the monthly dues and you may not receive any accumulated services.

Your subscription services are not transferable. They must be used by the subscriber enrolled in the program.

Appointments are made on a first-come, first-served basis. You are responsible for scheduling your monthly services in advance to ensure you receive your preferred date and time. If you are unable to keep a scheduled appointment, you must cancel at least 24 hours prior to your appointment or standard cancellation fees may apply.

Monthly Subscription dues do not include gratuities. Subscription services may not be combined with any other promotional offer.

Moonshadows Massage & Wellness, LLC. reserves the right to cancel any membership at any time.

A valid credit card are required to enroll in the Moonshadows Massage & Wellness, LLC. Twelve Month Subscription program. You agree to provide us with updated information if your address or payment information should change. If we are unable to process the automatic monthly subscription dues, the subscription will be suspended pending payment in full. If full payment has not been received within 5 Business Days your account will be considered cancelled and subject to the terms outlined above.

A fee of $30 may be required to reactivation the subscription. Any accumulated services will be forfeited if the subscription is cancelled due to nonpayment.

All attorney fees ($350/hour) and court fees required to enforce this contract will be the responsible of the subscriber.
